Scar Revison/Excision vs FUE

I am 32 and made the mistake of having a hair transplant when I simply do not have a enough hair on my head to fill my frontal or crown. Not enough supply for the demand. I should have paid more attention in economics class before undergoing the procedure. So, now I am stuck with the typical ear to ear FUT scar with a balding top of my head. I think I can maintain this for at least 10 years and look simply like a thin 30 something old.
My question is should I attempt an excision/revision or simply put FUE into my scar in attempt to trim it down to a respectable level all around (hoping for 1/4 inch or less with slight scar showing only)? My scar is 2-3 mm wide all around. I know there are plenty of you out there that have had this decision to make and it either bit you or you were successful. Maybe it is not worth the risk for the possibility of a 50% improvement?

Hi and welcome to the forum. Sorry to hear of your predicament

Supply and demand are your problem as you are aware, you say you`re continuing to lose more hair, have you tried Finasteride or Minoxidil to prevent the loss?

Is there any chance you could post a picture of your donor scar? This way we will be in a better position to give an opinion on which option we would undertake on ourselves if in your situation.

I am on both finasteride and minoxidil to slow my hair loss which I truly think has helped and may give me another 10 years of wearing my hair as it is (fingers crossed). I have posted a pic of my scar but you will not be able to see sides all that well. The middle is the thinnest at about 2 mm and it goes to 3-3.5 toward the sides.
Let me know what the rest of you guys think?

Looking at your scar it all depends on that you want to achieve. If you`re wanting to add more hair to your previously transplanted area I would personally try a scar revision using the grafts obtained to add to the your thinning area, laxity permitting of course.

If you`re not wanting any more hair transplanted I would go for Fue into the scar allowing you to have a shorter hair cut which imo looks better on a naturally thinning head.

Or you could excise certain segments of the scar and have the grafts placed into the remaining parts, not sure how viable this option would be but you could check into it with a doctor.

All great suggestions. I will keep researching and asking the opinions of the top docs. Hopefully I can get a somewhat clear picture of what to do. Challenging due to it being a gray area in regards to MD recommendation. Thanks again BigMac.

You could also have a small section of scar tissue that contains no grafts excised out to see how it heals up. If it's good then do another section and so on. Just a thought.

Your scar is not the greatest, it's not too bad either. A mixture of beard and normal donor hair Fue'd in there would be my choice.

I would first go for non invasive procedures like scalp micro pigmentation

if not satisfied you can do some FUE hair grafting

The combination of both techniques is great and can vastly improve the scarring.

I agree with BM, it mainly depends if you want to add more to your recipient area....personally if not then i would combine both FUE and SMP to the scar.

I just did SMP on a client with a scar similar to this. He also had grafts FUE'd into his scar (a combo of scalp and beard). It appeared that some of the grafts did in fact grow, but it appeared sporadic. I did not see him post-op so I'm not sure where the grafts were placed.

Scars like this are very difficult, whether it's SMP or FUE. You have compromised tissue where there are various layers/depths and the vascularity has changed. When my client comes back for his followup I'll post some pictures. You can also see some similar scars on our web site.

Your current scar width is not very uncommon, but a second FUT excision may result in a wider linear scar over time and involve stretch-back. You would need to identify your goal(s) for treating the scar. A motive would be to wear a shorter hairstyle, and if so, you could consider FUE, SMP or BHT as viable solutions.
Research the results and go with the method that best represents your expectations and goal.

I too am wanting to address a linear scar.(although I now have 2) I am still on the fence as to how to go about it but I have pretty well determined that scar revision is NOT my choice. I believe that I will go with SMP and then if it is not what I want I will then choose to try FUE or BHT.

I'd love to hear back from this guy, although a long shot with it being several years ago, he has probably moved on.
