3200 BHR Crown (2.5K Scalp 700 Beard) Bisanga & Kostis

Hi all, I wanted to document my recent experience. Hopefully this will be useful to follow as covers a range of things I know people are interested in:

crown transplant, oral minoxidil user, combination of Bisanga AND Kostis and beard hair. So hopefully an interesting or helpful thread, and hopefully for me help me through the coming months of recovery and hopefully growth!


so the stats:

i am 37 I have had two previous FUE with Prohair Clinic Belgium (they now focus on SMP as the lead doctor retired). These totalled 6k grafts and covers front then the rest in two separate ops 10 years ago. These were successful from my perspective, but ten years on I had least any native hair I had then (not any meds until recently) and wanted another transplant.


Long story short I had several consultations remotely from recommended doctors and travelled to Belgium from uk in April for a face to face consultation with BHR. Having done allot of research BHR we’re my number 1 choice. Dr Bisanga was unable to be there (family emergency) and I was offered the choice to change date or see dr Kostis, I was happy to proceed with Kostis.

After having 6k grafts in my previous surgeries I was keen to understand what was possible and in a detailed consultation with dr Kostis these were my results:

Hair Character - Straight

Hair Thickness - Very Fine

Donor Density - 70/70/80 (average - 73FU/cm2)

Hair Groupings - Mainly 1 and 2 hair grafts

Pattern of loss - If no previous surgery, likely a NW7 progressive pattern

i was very surprised when The doctor suggested I had in the region of 4-4.5k scalp grafts in the bank and a good beard and 2 separate surgeries were discussed to address frontal third density and the crown. The crown bothered me the most and as all my hair in the front was transplanted hair (so pretty safe) we agreed on a plan of 2.5k scalp and 500 beard to address the crown. After this we would review to see what was achievable with the remaining donor. Oral minoxidil was recommended to reduce miniaturisation of my donor and strengthen overall all my hair ready for surgery.


with my very fine hair and hair groupings I knew that this is still an illusion of dentistry even at that graft number due to the actual number of hairs we’d get and diameter of each.


i won’t go through the whole day and build up but happy to answer questions, what I will say is Ian the patient liaison is superb and tbh a big reason for me picking Bhr as I had offered from several very good clinics (obviously and the drs and the rep was my no1 reason!) but he was so detailed and responsive. The in person consultation for me with dr Kostis was critical as if I’d have been told what I was told without an in person using microscopes etc I would not have believed it.


i was offered dr kostis and bisanga as a pair or you can have either on their own all at different price structures. This is made very clear. I was very comfortable with dr kostis and trusted the partnership which I believe is the package offered initially to most now.


on the day (Monday 25th July) It was not easy I won’t lie as was there from 730am until prob 930pm but the whole staff were amazing both drs and all the technicians who worked so hard with no breaks. The anaesthetic was allot more painful than I remembered! But it’s a means to an end! Bisanga I believe did all my incisions and kostis all the punching extractions. The same tech team is used regardless of dr choice which was a great point made by Ian as they play such an important role.


we ended with 2500 scalp and 700 beard and I could t be happier with my choice of clinic. Obviously no results and I have to trust the reputation of the clinic here.


id like to thank them all as hardly got to see them all before I left as it was so late but after spending 13 hours solid with them, they were lovely people and made me feel very relaxed and safe. We even had a few laughs at times!!

here are my immediate post opp crown and day after the donor. I will share all the official pictures from the clinic when I receive them and will share pre opp below as well again official pre opp will follow.

and breathe! If anyone wants details of the day etc I am happy to answer but didn’t want to give my life story.

Congratulations on your third surgery. It sounds like everything from the initial contact, the assessment and then the surgery went extremely well. Having 6000 grafts removed in your previous surgeries and still having 4-4500 grafts available is a good testament to Pro Hairs extraction and donor management.
If you’d been on meds years ago you may have prevented this surgery but that’s in the past now. Being on them now will hopefully improve and maintain your hair.
The work from BHR looks good. Nice spread out extractions and placement. The beard grafts will add some good density to your crown and I would say the mix ratio to scalp hair will help it blend in very well. Under the chin usually heals quickly too.
13 hours is a long time at the clinic but you know it took this long because of the care and attention to detail. I think the 3200 grafts are going to make an excellent difference to your crown. I’ll be following your progress with a keen interest as I like to see crowns evolve.

I`d like to thank you for sharing your journey with the forum. It’s a good write-up that I believe will help others busy researching. I’m looking forward to seeing the pictures from the clinic when you receive them.

ps, how long have you been taking the Oral Minoxidil and any sides?

No I agree Sabine at pro hair was top class and they did a great job for me ten years ago.

My beard believe it or not is pretty much unnoticeable already!

I’ve been on oral minoxidil since April with no sides at all and I’m pretty confident was already having positive impact on my hair prior to surgery I hd big shed but then you could see the hair was better quality so hopefully that will help my progress from the transplant.

This is my beard today just 6 days lost 700 extractions.

That’s good to hear no sides from the Oral Minoxidil and that you believe you were seeing improvements prior to surgery. Your beard extraction area has healed very well with no sign of extractions. When I had my beard extractions it was quite itchy for a few days but healed similar to yours.

Healing looks good at 7 days. Quite a few scabs in the recipient area that should start coming off with your first and subsequent hair washes.

First wash almost all scabs came off feels so much better now.

What a difference that hair wash has made. You’re healing quickly which is always a feel good factor after a HT.
With the scabs and excess tissue gone I can see how the grafts have been angled to create your crown swirl. I think this will look great once it’s grown out.

Well touch wood yes I was in a good routine of exercise 4/5 times a week etc prior and have a good diet but nothing extreme. Take the normal supplements most do and oral minox. On the hole followed the instructions closely drinks lots of water etc and sat on my arse for a week basically trying to de stress but the healing does seem better than my previous transplants, Bhr give you various things to use but I’m sure nothing revolutionary.

You must be excited seeing this and knowing the difference this will make.
I am but obviously am always nervous of the what ifs../what if it doesn’t grow what if my donor looks too thin etc etc like everyone but I trust the doctors who did it so just have to put that aside and believe in the process and not stress. I plan to go back into the office next week I’ve told most people I’ve had this done as I feel that just makes everything easier rather than hiding or making up reasons for my shaved hair or no doubt red crown when this hair falls etc

You’re a veteran now being on HT number 3. The nervousness will disappear soon enough. It’s good that you were able to let people know you were having a HT. You`re correct that it will make life easier at the office. Most will be curious and want to take a look at your head. You may be the focus of attention for a short while.

Your diet, exercise and the supplements keeping your body working well.

Official pre and post opp pictures came through today. Great to have these to monitor progress. Incredible to see the beard immediately post opp here as it’s completely healed today on day 11 tbh was healed in a week and looks like nothing has been taken!

Thanks for adding the additional surgery pictures. There is some scarring from the the previous surgeries but Dr Bisanga has managed to do a nice educated extracted pattern that should leave your donor looking good with a little length to the hair.
Beard extractions look clean and precise. Healing in one week for you is great. You won’t miss those grafts.
The placement in the crown looks clean and refined. Seeing the surgery marking that indicate the direction of your crown whirl that’s being recreated is great. The angle and direction will provide great coverage.
Overall, everything looks good and the healing is going as expected.
