Hello there guys, this is my first post on this forum and I would like to ask your help about hairloss. Attached you will find some photos of my skull in order to have an idea about the stage of hairloss I am facing. I wanted to ask your opinion about the ways in which I can fix this because it really affects my personal life, as I am only 20 years old. In a greek forum they proposed the use of finasteride pills and I would love to know your opinion too about this kind of solution. Looking forward to your replies!

Hello Nikoskar.
Welcome to the forum.
I`m sorry to see you`re hair is thinning at this young age. The first thing I would do is see your family doctor to rule out any medical conditions that may cause this. You`ve already been told about the use of Finasteride on the Greek forum. You can talk to your family doctor about this as well.
If I were in your position I would use Finasteride to stop the loss and maybe improve the hair. Even with the small chance of side effects, I`d try it.
There are a few alternatives such as Oral Minoxidil or topical solutions.

Thank you very much for the information, however do you think that finasteride will be a better solution than dutasteride? Because I saw some people proposing dutasteride too and I can't tell the difference between them two, because I don't have a lot of info about them.

Personally, I would start off with Finasteride if I go by my own experience. I`ve been using it since 2005 and it`s worked very well. For better advice though I`d recommend speaking with your family doctor or try to arrange an in person consultation with a hair restoration doctor. Even if you have to pay for a consultation, the diagnosis and advice could prove invaluable. If you can avoid hair restoration surgery by holding onto your hair then that to me is the best option.

Finasteride and dutasteride are inhibitors of the enzyme 5-a reductase (5AR). This inhibits the conversion of testosterone to dihydrotestosterone (DHT), the androgen that primarily causes male pattern baldness.

There are 3 types of 5AR receptor isoenzymes, with type I and type II being significant in the treatment of AGA. Type I 5AR is the principle isoenzyme found in sebaceous glands, sweat glands, and the scalp, whereas type II 5AR is present in hair follicles and the prostate. The main difference between finasteride and dutasteride is that finasteride is selective for type II 5AR while dutasteride inhibits both type I and type II.

Dutasteride not only blocks both types of receptors but also does so much more effectively. This may be a result of the much longer half-life of dutasteride compared to finasteride (~4 weeks vs. 6-8 hours respectively). Dutasteride has been shown to be 3 times more potent than finasteride at inhibiting type II 5AR and more than 100 times at inhibiting type I 5AR. Furthermore, oral dutasteride decreases serum DHT concentrations by up to 90% while finasteride only reduces DHT by 70%

I would say it's worth exploring the non surgical options first, you don't need to rush straight to a transplant, especially with you being so young. I'd recommend speaking to a Dr and get a proper assessment before starting any treatment, like Bigmac suggests.
