DESPERATE MAN BLUES

Sometimes, I think that some surgeons decided to make a contest of the worst hair transplant, and the badly done.
Here is the example of a 38 years old French patient. He had 2 hair transplants in Tunisia, made by Dr A… in Tunis. This former doctor of the “Hôpitaux de Paris » define himself as a « Specialist in hair transplants since 1994».
The patient had 2 surgeries, and the final result is a real catastrophe: a front hairline where large bouquets hairs are drawn up backwards… and as special gift two huge scars on the donor area. He has also contracted a staphylococci infection which left scar after-effects on the donor area.
When I first met him, during the consultation I was not far from thinking that the situation is desperate. Two solutions are considered. First of all, it is necessary to remove a maximum of unnatural and huge grafts with micro punches, especially the ones on the anterior line and particularly those whose orientation is really unnatural.
Then we should decide either to carry out a FUE, or to make a FUT. Finally we decided to make a FUT for two reasons. First of all, the laxity of the donor area was surprisingly good despite the fact that he suffered two interventions, the two scars and because of the after-effects of the infection. Then, because of the scar which was really bad, so it was better to remove a new strip so to reduce it.
Thanks to the very good elasticity, I could almost remove the whole scar, and the number of grafts reached was enough to cover several balding areas: from 1 to 4!
1848 grafts including 549 singles, 882 FU2, 360 FU3, 57 FU4 for a total of 3621 hair.
The FU1 (singles) were used to lower the anterior hairline of about 1 cm, and I spread them between some few bad grafts that were not removed. Most of the 360 FU3 and the 57 FU4 were placed on the central area to give to the frontal core more density. The 877 FU2 were disseminated on the sides and placed to arrive to zone 4. The pictures of the day of the intervention were taken on our clinic. All the other pictures post-op were taken by the patient, he regularly send us pictures.
The evolution of the surgery is excellent, I do not have any pictures of the scar but I know that it is not perfect, it widened on more or less 1 cm.
We are thinking about making a correction to it thanks during a new intervention: this time exclusively FUE. I will remove the large grafts, still visible, and maybe (if it is possible) I will try to increase a little bit more the density and to repair the scar by placing grafts on it.
Some remarks on the pictures:
Two series of pictures show the work completed on the left side of the hair front line, where the situation was the worst (large grafts removed via a punch of 1.3mm).
The grafts which were more or less correctly orientated, and on the middle of the head were not removed and I surrounded them with single or FU2.
The 6 photographs taken chin down show that the former anterior line is slowly vanishing as it is gradually being hidden by the new hair.
The situation after 124 days shows precisely how the old grafts are vanishing, and how the frontal line doesn’t seem anymore dense and now the central core is more relevant. The situation at day 148 (5 months) is very promising.

At five months this patient is looking great, the state his head was in with the previous work was appalling.

Thank you Dr Devroye for a very detailed write up and presentation of the patients photos.

Looking forward to seeing the next phase of his repair if it is possible.

The patient must be very happy so far.

Thanks bm.

Wow. This poor guy was certainly not in the best of shape. Shame. So far the repair looks great and he still has a lot more growth to go. He is going to look so much better. It would be nice to follow his case and see results at the 1 yr. mark and ofcourse if he undergoes another surgery. Thanks for a very detailed post.

I hate seeing these cases as I can’t imagine how bad this guy must have felt after those previous procedures, it’s criminal really. The French clinics seem to be hacks as that’s the second case in as many months. I take my hat of to you Dr Devroye for correcting his previous procedures this guy’s self-esteem must have increased ten fold. Good skills.
