Dr Taleb Barghouthi - 3863 - crown/midscalp and front third refinement

Hi all, I thought I would share my experience and details of my recent surgery with Dr Taleb Barghouthi working out of Westminster medical group London uk.

first of all a bit of history as this is my third hair transplant with the first 2 strip transplants being carried out by Dr.Feller in New York.

I first started noticing hair loss at about 21 (now 43), and started to do my research on this and other forums.

i started taking Proscar which had slowed/halted further loss but after a few years on that I felt I was losing ground and switched to Dutasteride up until around 2013 at which point we and my partner started to try for a child and I decided to stop the meds just to be and the safe side and regrettably didn’t get back onto the meds until April thisyear.

i am currently taking oral finasteride 1mg and oral minoxidil 2.5 mg daily with no side effects at all.

Transplant history:

Transplant 1 (Feb 2006): 2600 grafts strip placed into the front third.

transplant 2 (Feb 2008): 2800 grafts strip placed into the crown, midscalp and refinement to the front third adding density and a small temple closure.

Transplant 3 (Oct 2024): 3863 grafts fue into the crown, midscalp and front third again to add density and reconnect where I have lost native hair.



I am extremely pleased with the work carried out by Dr Barghouthi and the team at Westminster medical group and they made the 2 day op a very comfortable and friendly experience.

Dr Barghouthi did all of the extractions and incisions and the 6 technicians worked in teams of 2 to add the extracted grafts which I thought was a great way to do it as they would do 20 minute stints as to not get too fatigued.

To finish it has been a long awaited surgery which I have been wanting to do for the last decade, but with kids and a busy schedule I thought it was finally time to get it done.

Hi Pete2, I hope you're in good health etc. It has been a long time since I've seen you on the forums. Dropping the meds is a common thing quite a few people do, and then regret it later. Hopefully, you can regain some ground with your new regimen.
Dr Barghouthi`s work looks very good. Getting nearly 4000 grafts this time is excellent. The placement looks good, and it should restore you to a full looking head of hair. It is good to hear the whole process went smoothly.
I see your strip scar stretched a bit, mainly on the right side. Was it a tight closure as i can see some of your follicles/hairs are growing at slightly upward facing angles? Have you any plans to address the scar?
In your pre op design I see they marked out all the way along your hairline but only the middle was transplanted. Did the doctor have a change of placing strategy for you?

Hi Bigmac, yes it’s been quite a few years indeed since being on the forums.

Yes the strip scar from my second strip had stretched a fair bit and we did talk about either doing a scar revision or adding grafts into the scar.
On the day we decided to leave the scar as 1. By doing a scar revision it could possibly increase the width and secondly I didn’t want to waste circa 400 grafts into the scar via fue as I don’t cut my hair short enough for it to show and I can generally get away with a no5 guard with no problems.
Although I am going back soon for smp into the scar to camouflage it some what.

As far as the hairline I was happy with the density of the hairline except the centre forelock as with my previous ht’s only a few grafts were placed into this area as it was still fairly strong back then, therefore I only ever wanted the central area and where the temples had become disconnected sorting out to evenly distribute my remaining donor throughout my mid scalp and crown.

Smp into the strip scar will help conceal it, maybe some beard grafts as well. I had beard from under my chin put in one of my scars, they grew in great.
Sounds like the grafts were distributed wisely. I think you will be happy with the result.
If you can remember, how do you compare FUE to your strip surgery?

I may go down the beard graft route for the scar but I will see what the outcome of the smp is and go from there.
From memory there wasn't a great deal of difference between my strip and my recent fue op other than the healing is much better/quicker with fue for obvious reasons.

Sounds good. Let us know how the SMP goes once you have it. I was similar to you, the FUT healing wasn't an issue.

Hi Rolie, sorry yes they are a bit misleading but the last 3 pics are pre op pics taken approx 2 weeks before my op.

Hi guys, I am now just over the 2 week post op point and all is going very well.
i have attached some current pics now that most of the scabs have been washed away.

i will be having my first session of smp added to theprevious strip scar next week so i will add an update of the outcome once completed.

Hi Guys, I have just passed the 2 month mark and I am well and truly in the ‘ugly duckling’ phase!
I have just had my second smp procedure completed into an old strip scar and I am very happy with the outcome (slightly darkened pigment at the moment but it will settle down in a week or so) I have also trimmed the back and sides to a number 3 guard to see how far I can go without seeing the previous strip scar from my surgery back in 2008.
all in all happy so far and I cannot wait for this phase to be over.

Healing looks right on track. This SMP session looks very good. The scar is concealed very good. Thanks for the update.

Hi guys, it will be exactly 4 months tomorrow since my ht.

All going ok and I am starting to see some progress.
I feel on the whole I am a slightly slow grower but it is improving month on month so I just need to be patient.

Looking good at 4 months. The growth looks pretty even. Your donor has recovered and looks good at the length you have it. I`d be pleased with this progress so far.
