I feel like I'm doomed. That's my story.

Hi guys. 23 years old male. I'm balding since 18 years old.

For years I've only used topical Minoxidil. Then added oral natural dht-blocker and topical antiandrogens. About six months ago I quitted the natural blocker and started a topical with:
- Minoxidil 3%
- Finasteride 0.3%
- Progesterone 1%
- Hydrocortisone butyrate 0,1%
- Ketoconazolo 0,5%

Mid and vertex are full of micro hair that no longer grows. They are probably already sentenced to death. Here how they do like when falling:

I think the damages in the front and mid are irreversible without an hair transplant.

I need to cover all my head with the topical, so I use more than 2ml. However, the topical contains high Finasteride dosage so to reduce the likelihood of sides I'm currently I'm applying it every other day. Given the disastrous situation, shoul I go for a daily application instead?

What else can I do to boost the cure? I'm evaluating:
1. Topical Dutasterine instead of fina
2. Scalp massages
3. Microneedling
4. Botox ininjections
5. Prp or Prgf
6. Natural orally anti-dht (I'm afraid of sides considering the fina lotion is high dosage)
7. Sulforaphane
8. Procyanidin B2
What else?

I assume you're ruled out oral finasteride / dutasteride, which would be more effective?

Actually none of the trichologists I had a visit with have ever prescribed oral formulation of Finasteride, let alone Dutasterine.

However, I would really like to avoid as much as possible the systemic impact of these drugs.

Given that, topical seems a reasonable approach but I guess my situation is too advanced and I may need a transplant to restore the front and mid.

Have you read about the potential additions I'm considering? And what do you think about applying the topical every day instead of every other day? (I've explained above why I'm avoiding everyday application)

Given that your loss covers a large area and you're relatively young, I don't think you have the luxury of being overly cautious with medication.

If you don't get to the standard daily dose of oral finasteride I believe the other methods you have described will not be effective in halting your loss.

If you cannot stabilise your situation, a hair transplant in a few years may not be possible, or least not to maintain a full head of hair.

Sorry to be the bearer of bad news, but I really do think you need to change your approach, and quickly.

My topical contains a very high dose of Finasteride (0.3%). In these 6 months I wanted to be cautious and I've never applied the suggested dose of 2ml per day.

Doing so is equal to put 6mg of Finasteride everyday in my scalp. Therefore, I started with 1ml everyday but realized that I need to cover almost all the head. This involved putting more than 2ml per day, probably 3ml to 4ml. Therefore, I switched to a every other day application to avoid putting the equivalent of 18mg to 24mg of Finasteride in my head within two consecutive days.

I may go for the suggested dose of 2ml daily and try to cover as much as possible. I think such an high topical dose may be equivalent to an oral intake.

If it were the same, you'd likely get the same symptoms as taking it orally. Anyway, feel free to ignore me. All the best in your journey.

I think u should go to a good hairloss clinic that will check ur hair etc and put u on meds
Personally I think u should go to dr Edward ball maitland clinic uk who also have a in house tricholoist
U need to go on either
Oral finesteride and oral minoxidil
Or oral minoxidil and oral dusteride
Or just oral finestide
Yes there are side effects but u are worried about ur hair
If u don't go on meds I would just cut ur hair short and try 2 move on with ur life
Trust me I know what I'm talking about as it will start to destroy ur life

Oral Minoxidil seems an overkill to me. I may start oral finasteride and stick topical Minoxidil.

Are you current on medication or did you give up?

I was on oral finesterde long time ago but yeah just thought it wasn't working so stopped, how wrong was I,started to thin
Then over the yrs more and more hairloss treatments came out just got confused with them all so just stopped everything
Deffo knock topical products mate it will dry ur hair out
Best to go see a dr in the UK if that's where u live ie dr ball or ur choice
So they can keep eye on ur hair and help
U probably have to pay consultant fee but be best money u pay to just basically get u in the right direction
