This is a 25 year old who came to us a while ago, concerned with his present loss and future progression.
On no form of treatment.
What would you do?

- fine hair

I would explore the option of medication to try to halt / slow his loss, and pray for some regrowth. I don't think surgery is the right option for him (or at least not yet).

Definitely a medical approach first (Fin(Dus)/Minoxidil/Ketoconazole) to try and stabilize the hair loss. Reassess in a year assuming loss has stabilized and no sides but even assuming it became stable I would still air on the side of caution and be conservative because of retrograde alopecia ?

I don't think merely giving medicines (Finasteride-Minoxidil) is correct.

1. The patient has (most likely) lost hair starting at a very young age (not the age at which his predecessors lost hair).
That means, he has kick started his hairloss genes (which were in sleep mode since birth), by turning his Epigenome unhealthy.

Most likely, if you ask him, he will narrate life events preceding the start of his hairloss (change of residence, going to hostel/college, change in diet, water, air, stress levels, severe illness etc.).

Those are the epigenetic events that start the hairloss process.
Advise him to improve his lifestyle, take healthy supplements like GlyNac, NMN, Omega 3, Quercetin etc.
Start once a week microneedling accompanied by daily massage of Resveratrol Grapeseed extract serum into the scalp.

2. Start him on GHK Cu and TB 500 peptides intradermally in scalp.

That should suffice in slowing down/reversing his hairloss.
If you want, you can add the Minox/Finasteride combination alongside, but I don't think it will be necessary.

Once his prematurely activated genes of hairloss are quitened, then the area requiring transplant will be much less.
In my experience, once the patient notices improved hair growth without drugs/medicines, he will have a different outlook and you will be able to convince him to delay a hair transplant.

He really doesn't look like a candidate for a hair transplant so as others say non surgical treatments should be used. You mention in your post that he came in a while ago, so you are going to post an amazing response to medication next?

I hope so!

If it was me I would just clipper it short say no 2 guard or 1 guard
Grow some stubble if u can
Trust me u hair is fine texture so u will get a comb over apperance with transplants, u will save alot of stress and money and heartache if u went the transplants and meds route cos u won't get results u want.yeah its not want u want to hear but u will be so much happier if u just accept it and try deal with it by cutting it short,know its hard mate,I had my 1st transplant in 2003 I was 23yrs old, I've had another 2 in 2008 and 2010 so it's effected me for a while and I'm not happy, all I had was a receding hairline so believe me transplants or meds ain't gonna get u result u have in your head
As u get older u won't care as much

Dont know about that, Im 36 now and still care equally the same if not more than when I had my first one at 24. I think oral minox, finasteride could do wonders.

Exactly, u had a transplant and u still thinking about your hair so to me it's not giving u what u want so u can move on and not worry about hairloss

Im happy with what I got, no question about that. Im not on here worrying about mine, more a case of helping others as I have been through it.

I'm happy for u mate,I'm just trying to point out especially in the guy I'm talking about ,I think it's a bad idea to have a transplant or even meds cos I don't think he will get any benefit from it,maybe keep what he has, but he will do what he wants so good luck to him
I've been on the forums on and off for about 16 yrs I've heard all the new methods,new meds etc etc etc, and nothings changed that much really, that's my opinion, and I've also had 3 hair transplants,2 with a so called top dr and my hair looks shite,but guess I'm one of the unlucky ones like the thousands out there
I'm happy u got what u wanted mate really I am,I just think the hair industry is a sham and u never really get the results u want,u just end up accepting the result which in turn convinces u into thinking its a good result,I reckon alot of guys would agree with me on that
Not hating on nobody mate,I just think it's just a nightmare after having ur 1st transplant u are then just in a rut fighting it

Of course everyone is entitled to their opinion and we have different journeys helping us form our opinions. I was on a poor journey after having two FUE which seemingly thinned over time. A but of magic from Dr Farjo changed things completely. The HT industry can be tough. have you seen how thin my front was? oral minox with no surgery on it in the past years thickened it up massively so they can work.

Finasteride and oral minoxidil for a year and see what the meds do by then.in the meantime cut it short shave the back and sides down and crop the top,it will look far better less is more in this situation.i get a lot of clients in this situation and once I’ve cut it short and they have time to adjust to the new style they are happier.Not saying they don’t want a HT still but try the short hair first.

I completely agree with you - it's great that he is happy with the outcome, and I hope that everyone on this forum experiences positive results as well.

Some people do respond very well to the meds. If they don`t try them, they’ll never know.

I was thinking about this post. I think if he were to have a positive response to medication then restoring a conservative forelock with surgery could be an appropriate course of action.
