Hair loss caused by burn scars is one of the most challenging conditions in the field of hair restoration. In many cases, patients are informed that no treatment options are possible because scar tissue often has poor blood supply and reduced graft survival rates. However, with proper planning, advanced techniques, and careful execution, successful hair restoration can still be achieved in selected scar cases.
At The Hairsmith Clinic™, we recently treated a patient with a significant burn scar on the scalp who had previously visited multiple centers seeking a solution for hair restoration. Unfortunately, several plastic surgeons had declined treatment, informing him that restoring hair growth in the scarred region would not be possible.*
This case study highlights how a carefully planned approach using our Advanced UR-FUE™ Hair Transplant Technique helped achieve successful hair growth in the burn-scarred area.
Patient History:-
The patient aged 24 years, presented with a burn scar since 10years after an electric shock affecting a visible large portion on left side of the scalp. Due to the scarred nature of the tissue, conventional treatment approaches were considered difficult by multiple centers the patient had previously consulted.
The primary concerns included:
Our Treatment Strategy
Rather than proceeding directly with a large procedure, our team decided to take a scientifically planned step-by-step approach.
Phase 1 – Sample Hair Transplant (100 Grafts)
The first step involved performing a small test session in the burn-scarred area.
Objective of the Sample Session
The purpose of this initial procedure was to evaluate:
Result of the Test Session
The transplanted grafts demonstrated successful survival and healthy hair growth within the scar tissue within 6months, though the final density result comes in 12 months. The outcome of this initial phase gave the patient confidence to proceed with complete restoration.
The before-and-after images from this stage showed promising improvement and confirmed that the scar tissue was capable of supporting transplanted follicles.
Phase 2 – Full Hair Transplant Procedure
Following the successful outcome of the test session, the patient decided to proceed with complete coverage of the remaining burn-scarred region on Jan 2026.
During the comprehensive procedure:
Final Results – 4 Months After 2nd Procedure
The final results observed at 4 months post-procedure showed significant cosmetic improvement and the density will improve further.
Outcome Highlights
• Successful hair growth within the burn-scarred region
• Improved scalp coverage
• Natural appearance
• High graft survival in scar tissue
• Enhanced patient confidence and satisfaction
The before-and-after photographs demonstrate the transformation achieved after the full restoration procedure
Why Burn Scar Hair Transplants Are Challenging
Hair transplantation in burn scars is technically demanding because:
At The Hairsmith Clinic™, every scar restoration case is evaluated individually to determine the safest and most effective treatment plan.
Importance of a Test Patch Procedure
One of the most important aspects of this case was the decision to begin with a small sample transplant.
This approach helped:
Conclusion
This case demonstrates that even complex burn-scar hair restoration cases can achieve successful outcomes when approached with proper planning, advanced techniques, and realistic expectations.
Despite being informed by multiple centers that treatment was not possible, the patient achieved visible hair growth and improved scalp coverage through a carefully staged treatment plan at The Hairsmith Clinic.
Using our Advanced UR-FUE™ Hair Transplant Technique, we were able to successfully transplant hair into burn-scar tissue and deliver natural-looking results.

The primary concerns included:
- Lack of hair growth within the scarred area
- Cosmetic appearance affecting confidence
- Concerns regarding graft survival in scar tissue
- Fear of unsuccessful outcomes due to previous consultations

The first step involved performing a small test session in the burn-scarred area.
Objective of the Sample Session
The purpose of this initial procedure was to evaluate:

- Graft survival potential
- Healing response
- Hair growth quality within the scarred scalp

Phase 2 – Full Hair Transplant Procedure
Following the successful outcome of the test session, the patient decided to proceed with complete coverage of the remaining burn-scarred region on Jan 2026.
During the comprehensive procedure:
- A total of 511 grafts were implanted
- The procedure was performed using the Advanced UR-FUE™ Hair Transplant Technique by experienced team of doctors and technicians under the supervision of qualified registered medical practitioner.
- Special care was taken to ensure proper graft placement, density planning, and minimal trauma to the scar tissue
Final Results – 4 Months After 2nd Procedure
The final results observed at 4 months post-procedure showed significant cosmetic improvement and the density will improve further.

Why Burn Scar Hair Transplants Are Challenging
Hair transplantation in burn scars is technically demanding because:
- Scar tissue often has reduced blood circulation
- The skin may be tight or uneven
- Graft survival rates can vary
- Previous injuries may affect tissue quality
At The Hairsmith Clinic™, every scar restoration case is evaluated individually to determine the safest and most effective treatment plan.
Importance of a Test Patch Procedure
One of the most important aspects of this case was the decision to begin with a small sample transplant.
This approach helped:
- Assess graft survival safely
- Reduce patient risk
- Build confidence before a larger procedure
- Create a predictable treatment roadmap
