That's one big decision you have ahead of you. I know you have already given your reasons for not taking Finasteride orally. But have you researched applying it topically as a liquid. I seem to remember many posters on Hairlosstalk (before I got banned) using this approach.
That's a big difference in graft estimation, I'm assuming Bisanga's estimate is based on the fact that the worst case scenario is you may become NW7 in later life.

NW7 while father + grandfathers are NW5 at 51, 77 and 84 years old. I would be the unluckiest guy in the world.

Thanks for the suggestion but suffering from gynecomastia, even for 6 months was very traumatic, I won't risk it even topically.

I'll see, I'm ok with my appearance right now, but I will never accept to have a tenhead and it could happen in the space of a few months with my pattern, NW7 or not. I could avoid it with this first HT, I see guys who are NW7 (well where the slickness goes way down to the back behind) but still have somehow some hair a the hairline, and it makes all the difference in the world.

Bisanga didn't agree with me on this but I still think some hair (as long as it looks natural) is always better than no hair, always. Something like this, below.

Way better than being slick in my opinion for a white guy like him or me. I had so many chemotherapy comments when I tried to shave it completely.

I know it could just fail. Like my PRK surgery could fail, or my gynecomastia surgery could fail. But it didn't, I made sure to choose some of the Europe's best surgeons in the area. I just took the plunge. I'll keep you guys updated.

Totally agree with you on the hairline issue, the framing of the face makes such a difference. With the back and sides of the head no longer than 3mm, it can create a good illusion of density on top with the hair on top being 12-15mm at the hairline and getting shorter as it goes back.

Topkat, great advice!! You seem to be aware of hair transplants and the options people should choose, I have this awareness also and have been researching over 10 years, I have changed my life for the better. Would be great to skype and share knowledge. I have had 4 hair transplants to date and designed my own, but I did go to the best hair restoration clinic. Would be great to speak with you.

The only problem with just framing the face via hair transplant is the fact that the transplanted hair will be strong and dark and may look unnatural, whereas the guy in the pic above is clearly thinning where he has hair so it looks natural.

It may look unnatural, but I've seen De Reys' work on people that were NW6 (so the whole bald area could not be covered)and it looked quite natural. The question now is: is it more aesthetically acceptable to be slick bald and look natural or to have a slightly unatural frame to your face?

I think I would choose the frame, and if it really does look so awful, I could just laser the trasnplanted hair and go back to being a nineheaded chrome dome.

With 2k+ grafts, not going over the crown, and your light hair characteristics, do you think you could establish a N3 type hairline? Kiefer Sutherland comes to mind for an example.

I've thought about doing that. Jack Bauer really looks great with that NW3, but I don't think it would work on me. I'm not sure as I've never been a NW3. I started with a NW2 hairline, and then it faded to my current NW5.

Dr. De Reys drew my hairline way too low when I first visited him. Some people like me were just born with a very high forehead. That's why I don't think I could go higher, it was already pretty high during my teens.

Did Dr Bisanga mention the possibility of using body hair as a donor source?

If the contrast between hair colour and skin colour is low it will look natural, how is your beard donor? Maybe using all available head and beard fue and getting smp might be a good option. One could also harvest more scalp fue than normal and smp the donor after too..

No, Bisanga didn't even mention BHT. It's like I have the worst case of MPB ever and I will end up a NW9 or something. It's not the case but that's what I feel. Finasteride is not an option me, hair transplants are not an option, SMP is not an option, BHT is not an option, hell even wigs aren't an option as I want very short hair. I'm glad I can at least use minoxidil.

I have quite a thick beard under my chin, and I wouldn't mind it being harvest as donor.

Now SMP alone wouldn't work, but SMP with FUE('s) done before might work. That's the main reason I can't have SMP, the constrast between the skin and my hair is quite low when shaved. Thank you for your suggestions.

I think it's impressing some surgeons or clinics that I started balding diffusely at 17 and they think it's so bad for me at almost 24 now. It is not. I'm not obsessed with my hair right now, I really like how I look. Especially since I've discovered Milk of Magnesia to remove the shine on my head. But I know I will reach a point when I don't have the illusion of some frame to my face and it will make me really depressed. My goal is to avoid that altogether. But the less hair I have before the HT, the better of course.

I will have to move my first FUE with Dr. De Reys to January as I'll have an internship from October to December. I'll have more time to think about it and see how it goes for me with even less hair.
