Limiting discomfort during FUE procedure

Although FUE is more like a €˜treatment €™ instead of true surgery, there still are some factors that can cause discomfort to the patient.

In particular we are aware of 2 issues that come up most :

1. Discomfort during local anesthetics

2. Longer periods of uncomfortable positioning during extractions

Here are some solutions we have incorporated in order to manage these discomforts:

Local anesthetics:

Ice is used to cool the skin nerves before the actual local anesthetic is administered. Ice is known to slow down pain signals and helps to reduce the overall discomfort

Pre-administration of local anesthetics using non needle applicator. This device €˜shoots €™ some local anesthetics through the skin under high pressure, no needle is necessary. The lighting fast injection is so short that the €˜pain €™ is gone before we realize it.

Vibration applicator to distract nerve signals: hundreds of vibrations in a small area of the skin tends to €˜confuse €™ the brain, filtering out some of the pain sensations.

In all, when combined we find that most patients report us a 1-4 discomfort level on a scale of 10 (10 being most painful).

Shortening treatment time:

FUE is time consuming as it requires manual handwork for every graft. After extensive research and trials we no longer strive to use mechanical aids during the extraction period as we find that it yields in lower extraction quality.

Our solution to speed up extraction is to use left-right dual extraction. These extremely trained extraction specialist are capable of delivering particularly high quality grafts using manual procedure. We believe that our team is probably the first team worldwide to adapt this new extraction protocol successfully on a daily basis. It also helps to decrease the stress levels of the persons performing the extractions by spreading the work load.

The goal of our protocol is to cut patient €˜in chair time €™. Obviously it is a process that needs to be learned over time, which we have since early 2010. Our new protocol enables us to harvest manually at speeds of over +700 grafts per hour without decreasing extraction qualtity, making FUE mega sessions more comfortable (and affordable) for our patients and still achieve very high quality levels.

The only pain i've had with fue is the initial pain killer injections. I've had discomfort during the procedure because my body seems to metabolize the painkillers very quickly.

There's only so much "top-up" the doc could give me because all that fluid has to go somewhere and it made my head swell up. Even so it was still bearable enough that I could fall asleep.

Hi Sparky,
I had a test done on myselve by extracting a graft without local anestethic.

If you ever have been touched by the tip of a burning cigarette, multiply by factor 10 ... that is more or less how it feels.

Feet are usually the first give away when patient start to feel discomfort. Some guys try to bite the pain, but the jerky feet movements are quickly noticed.

Take care,

Got touch by the tip of a cigarette when I was 11, by accident but the 40 year old who did it didn't apologize, my Dad soon rectified that though.

Wasn't too bad, but x10 would be nasty.

The painkillers started to wear off towards the end of placement, didn't feel anything during extraction.

Interesting post. I like the idea of the 2 extraction specialists to limit the chair time. Makes the most sense to me and obviously it allows for larger sessions in a day. Ofcourse it increases the expense to the clinic which is a consideration.

More and more I like the idea of Fue. Like any procedure there has to be a lot of forethought with harvesting and also a person needs to seek out only the most reputable clinics demonstrating top notch yields consistantly.
