Options for Diffuse Thinning.

Can someone tell me what the bottom line is on diffuse thinning ?


Most information suggests you should not have a hair transplant and simply try Finasteride and Rogaine.

Has anyone any experience with diffuse thinning and being told getting a HT is not the right move ? If so what was your next move ?

I would really like to address my thinning hair, mainly the frontal third with a natural hairline.

Dear Haribo44,
I do not think there is a bottom line for diffuse thinners before they may opt for a hair transplant.
1. It is a difficult situation for the doctor as well as the patient as the speed of miniaturization and eventual hairloss differs on an individual basis.

2. Medications may or may not benefit all patients.

Therefore, my suggestion for people with diffuse hairloss is (medications apart), is to go for tricopigmentation. Not the permanent SMP, but the temporary one. I have seen a lot of very good results and it can act as a stop gap solution for your situation.

I'm glad Dr. Poswal mentioned and referred to tricopigmentation because I was thinking the same thing. In late 2013 I had half of my crown addressed with tricopigmentation to show the benefits of it's use at the 2013 ISHRS conference. In the video below you can see a direct comparison of one side with and the other side without tricopigmentation. My crown is fully transplanted (along with the rest of my scalp) and the density would resemble that of some diffused thinners so it could give you an idea of it's efficacy.

I'm glad Dr. Poswal posted the info about tricopigmentation. I believe it works well on diffused thinners and has the potential to make a real difference.
Below is a video shot of my scalp right before and after the 2013 ISHRS conference in San Francisco. With just about 10,000 grafts transplanted my crown is still thin (worse when wet) and it would resemble what some diffused thinners have throughout their scalp. Tricopigmentation made a huge difference as you will see...

Hi Joe, the Smp looks good. How long did it last ? I'm more concerned about addressing my hairline so I don't know how well Smp would work. Ideally I want something permanent but Smp may act as a stop gap until I decide what to do.

As long as the hairline is done right it can look good. You can't have maximum density brought to the very front of your existing hairline else it will look off. The density should be gradually increased as the smp is placed further into the hairline. The impact is not as great as seen in other areas but it can offer a nice boost.

The result you saw in the video lasted for a few weeks till the initial final result settled in, which was close to what you saw in the video. There is some fading in the first few weeks but this is normal. The result then lasted for about a year and now it is pretty much completely gone. I'm sure there is some residual remaining but not enough to make a cosmetic difference. Keep in mind since I had the procedure I was working out a lot, four days a week for at least an hour a day, so that may have accelerated the fade.

Joe did you document the monthly changes in your Smp ? It would be helpful to see the different stages after each treatment and the way it fades.
Are you planning on having Smp again ?

No, I did not. I do have some photos taken at various times since, and some are of my crown, but it would not represent an honest comparison since they are outdoors and under completely different circumstances. If i see some that are closer in comparison then I'll post them up.

When you go for consults see which ones let you know that you could lose most of your hair even though you are using meds, you may not have enough donor to keep up with demand if you need multie procedures down the line.
