Revisiting an FUE problem...

So here is a second patient that has come to us this year with the same problem. I posted the original problem in a different post... Here is an excerpt from the previous post...:

"... Basically, this was a person that had a very minimal hair loss issue however he wanted to address it...

... So he was looking to shore up the right side and be done with it...

... Here was the the over looked issue... The patient's frontal third and hairline is light in color. His donor area has light and dark grafts mixed.

... The person performing the FUE extracted the grafts that were the easiest... which were the dark and course ones...

... well one can see that in my crude drawing you can imagine what that left him.

... A patch of strong grafts in a corner of an area where the surrounding area is light... "

Now on to the second patient that has this exact problem...
Except this time I took a picture of this patient's grafts under a microscope to further illustrate the issue.





As you can see the two grafts on the left are light in color... pointed out in red. The two grafts on the right are very dark in color... if not black.

This patient had both his right and left temporal ressions transplanted with the dark course hairs. Everything behind the transplant is light in color.

Great picture that, it truly demonstrates the difference in hairs. Was the plan of action for this patient the same ?

Its happened to me, my grafts were alot more courser than the grafts from smg..how the hell does this happen it makes no sense to me?how can hairs change?

This seems so basic yet many clinics will place those thicker caliber hairs in the hairline in fact it €™s one of the biggest drawbacks of those that only have a thinning hairline but are persuaded to have strip. One really does have to ask why this happens and the answer is because those clinics simply do not care enough for it to be a concern. Instead of opting for a better approach even if that means sending that patient to another clinic they will instead try and force a square peg into a round hole and convince themselves that it looks good.

@bullitnut... thanks... Well to combat this as a repair we went for more of the lighter ones and softened the harshness of the previous work. *nl

@mars... In my opinion the hairs aren't changing. The darker courser ones are easier to see therefore the clinic might be going for them only. It would be better to grab a mixture of both and definitely try to match the front better. Many times the donor area is darker than the top. ))N_)

Good post and an often overlooked topic. The Hair line doesn't only need singles but the correct ones and not chunky ones from the occipital area but the finer ones found at the sides and above the ears. The hair coarseness changes throughout the scalp and with good reason, so one needs to place wisely in order to create a hair line that is not a mass of thick hair, beit even singles.

If Strip or FUE targets these areas for harvesting a hair line then there will undoubtedly be issues later on. Some will split multiple grafts into singles but again if they come from a coarser part of the scalp then the result will not be optimum.

Getting back to this topic... yesterday we had a patient who had salt and pepper hair. He in all seriousness asked if we could only choose the dark hairs for the transplant. Dr. Paul had to explain to him that this would not be a natural look especially since we were doing most of his frontal 1/3rd.
