What is this?

I'm 4 months post transplant. While I was rubbing my scalp to apply topical Finestaride/minoxidil tonight, I felt this small dry skin and realized it was a hair strand with a bulb at the end. Is this a graft? There was no blood or anything like that.

It is a shed hair shaft with some dead tissue attached.

I would say it was a stalled hair. It grew for a short while after it was transplanted, then died and just sat there until now.

Is that something that will regrow or should I count that follicle out?

It will regrow. Sometimes grafts/hairs that have shed do not fall out, they are then stalled hairs just sitting there. I had them on one of my crown transplants.
