can dermaroller give a false posittive for LPP (Lichen Planus Pillaris).

I recently had a biopsy done of my balding crown. to my dr. and my surprise it came back positive for LPP (Lichen Planus Pillaris). He said he was puzzled because normally people who have LPP have a rash and other stuff. he said I had a few pimples in the are but noting that looked out of the ordinary.
I used the derma roller on my crown for around 6 weeks. do you think that can trigger a false positive since it creates scar tissue?

I spoke to Dr. Shapiro about this and he said he'd never heard of a situation where the derm roller would give a false positive. However, Dr Shapiro mentioned that lab work isn't always accurate and number of factors can give a false positive result.

There must have been some sort of concern from your physician to order the test in the first place but another test may be in order. He says it's important to have a proper person read the test, not just a pathologist but a more specific derm-pathologist.

The results were read by my dermatologist. I recently consulted with a ht doc who said I had mild foliculitus. I went to my derm for treatment and also asked for a LPP test since I rember your post about getting one before a ht just to be safe.

I saw my derm today. He prescribed cortisone cream. I will come back in 2 weeks. he was shocked it was lpp. when he did the test he did it just to humor me since I brought it up.

Janna - how many cases of lpp do you normally see in your practice.
do you think we should ask the moderator to make a sticky post about getting an lpp biopsy immediately once you start noticing hairloss. I mean people always complain of mpb itch.maybe more people have lpp than thought

It's only been in the last 3 years that I became aware of LPP. In this span of time, there have been maybe 4 or 5 patients who tested positive for it. These patients had spots in the recipient area that transplanted hairs grew poorly.
I'm not sure if everyone who starts losing hair needs to be tested for LPP but maybe for those who have patchy hair loss or scalp irritations with hair loss? To me the percentage of people who have LPP seem very low but perhaps it's because many people aren't aware of it so they don't get tested for it. Let me check with our physicians if they think all patients should get tested before scheduling surgery. They may say for more selective patients, such as patients who exhibit specific signs they'd recommend it. I'll get back to this post on their thoughts on it.

update. the front biopsy shows no lpp so its only on crown. my doc said we can try cortisone shots

can grafts be planted in the frontal area where there is no lpp

We had a patient who had an FUE procedure where transplanted hairs grew just fine except for a small circular spot in the frontal right. He ended up testing positive for LPP when he biopsy'd tissue in the circular spot. In this case grafts grew in non-LPP areas.

I've seen some hair transplant pictures where they had LLP, the results were good. If I find them again I will post the link.

Dr Shapiro thinks the best route to take is to get another test done as he would not be able to determine if it's false positive from the result sheet.

What exactly is a false positive? If you have the test done race and it comes back positive is it safe to assume you have LLP
