I’m considering hair transplant in Turkey with Asli Tarcan. I know that they take max grafts and i’m afraid of overharvesting.

I got some questions.


1. How many grafts do you estimate i need? (with pictures)
Like how many grafts in each area on front and the crown?

2.Why do some clinics take max grafts?

3. How many grafts is considered overharvesting?

4. Any one with black curly hair who has done HT with Asli Tarcan?

5. Would 3500-3700 + 1000 beards cover my balding areas?

6. Does my donor area look it has good enough grafts?

5. Are people with black curly hair more prone to fue scars on the donor area? If so why?

6. I like having a shaven look at the back of my head. I know there will be scars but does it mean i can’t cut my hair that low after fue?

7. what is the best punch size for minimal scars?

8. What norwood scale do think fall in? 3 or 4?

Like how many grafts in each area on front and the crown?

At a guess, I`d say you need around 3000 grafts. 1500 in the frontal zone that includes your hairline. 1500 in the crown area. Your hair curl will help with the illusion of density.

2.Why do some clinics take max grafts?

Max grafts could be interpreted as using whatever number is required to accomplish the surgery plan. If they say they will fill all your thinning areas, then they may say this will take 4000 grafts. Therefore they will extract a max of 4000. It could be interpreted as they will use every graft they can safely harvest from your donor. You will need to confirm with the clinic what they actually mean with this term.

3. How many grafts is considered overharvesting?

Overharvesting is when too many follicular units are taken from the donor site.
Each patient has a different donor amount. Therefore the graft numbers will vary.
A patient with good density has more grafts available than someone with a thin donor area.

I`m not sure.

5. Would 3500-3700 + 1000 beards cover my balding areas?

Yes but I think that may be too many grafts judging by your donor and hair loss pattern. It`s advisable to have a few consultations if possible and compare

6. Does my donor area look it has good enough grafts?

From the pictures, your donor area looks to be on the lower side. Your crown looks as though it is beginning to dip down at the back towards your donor area.

5. Are people with black curly hair more prone to fue scars on the donor area? If so why?

Dark skin is more prone to hypertrophic and keloid scars as well as pigmentation changes at the donor sites. The smaller the FUE punch, the better chance you have of less visible scars.

6. I like having a shaven look at the back of my head. I know there will be scars but does it mean i can’t cut my hair that low after fue?

This depends on the skill of the person doing the extractions, the punch size and dependant on how you heal.

7. what is the best punch size for minimal scars?

As small as safely possible. Again, dependant on the skill of whoever does the extractions.

Norwood 4, possibly heading towards a 5.

Ask Asli Tarcan why they say on their website there will be no scars after a hair transplant.

9. Does afro curly hair give less or better cover after HT? Is it more difficult to do HT on curly hair?

Curly hair usually gives better coverage.
Curly hair is more difficult to extract because the graft curls under the skin in a C shape.

I'm realistic and know I'll need a 2nd HT. I also do not expect to get my hair back the way it was when I was 15. I just want to cover my bald areas. I also want less grafts as possible.

Your English is good.
Ask the clinic to explain how many grafts will go into what areas. What density will they plant at?

Are you on any type of hair loss prevention regime? If you get a hair transplant, you may continue to lose more of your native hair.

@Bigmac What i meant by max grafts is that i have seen HT patients talk about that this certain clinic always is trying to harvest more grafts than needed. Like why would they do that? That would be just more work for them and someone who is over harvested would most likely not do a 2nd HT.

I expect more hair loss and to norwood 6. I’m 31 and had slow hair loss since i was 21.
I have done some research and i was somehow looking for confirmation.

Some clinics, especially cheaper hair mill type ones, might over-harvest from the donor supply for a number of reasons. Namely due to the fact that they often completely fail to plan ahead for the patients future hair-loss issues and attempt to fix only the current state of your hair as it is today. Combine this with a lack of skill and or absence of a qualified doctor performing the extractions and you might end up with a lot of transection (where the extractions are done poorly and essentially kill the hair follicle upon removal), meaning that more extractions have to be made. An example of this might be that you have 5000 FU extracted, but only 3500 survive due to poor technique, so essentially a huge over compensation. The mindset seems to usually be one of just give the patient as many grafts as possible so that they have the best chance of acceptable/good results when using a really poor, rushed technique if that makes sense.

Max grafts for over-harvesting are going to completely depend on your donor characteristics - density, hair calibre, FU groupings (how many hairs you have per graft on average) and so on. As long as extractions are carried out properly with enough spacing and in the correct pattern etc, anywhere from 5 to 6, 7 or even 8 thousand grafts would be in the 'average' range (although I believe density is generally less for afro hair but could be wrong), with higher amount I just stated maybe exceeding average and into above-average territory. But even so some people have less than this and some lucky people even have more than this, so it depends on the person.

Having a consultation with a good/ethical local doctor would definitely be a good idea, just to get some accurate measurements of your donor, hair characteristics etc so you know where you stand before diving into surgery.

I don't know too much about Turkish clinics really as I have never really considered them too heavily, but whilst there are certainly some really good ones here and there, there are many more that could end up over-harvesting you, disfiguring the back of your head and leaving you much worse off than you are now, so just bear that in mind and don't make any rash decisions you might end up regretting for the rest of your life. I have coincidentally seen a couple of videos of some black guys who went to Asli Tarcan on YouTube, and from what I could see on their donor areas, they looked completely over-harvested with a clear pattern showing months and months down the line, so I would look into that a bit more and come to your own conclusion.

As for doctors that I know are good with afro hair, I know that Dr. Bisanga, Dr. Mwamba and Dr. Lupanzula (all in Belgium) are all considered to be experts in the field and they are all very ethical, renowned doctors who you don't need to worry about being over-harvested etc with. I would get in touch with all 3 and ask for their opinions. I've also heard dr. Diep in the states is supposed to be good with afro hair, but I don't know too much about him. Most of all don't let price sway your decision too much, save up more if you have to.

The most concerning thing in my opinion though with your case is your pattern of baldness; it is absolutely clear that you are eventually going to end up in Norwood 6 territory, maybe beyond. Your crown has already miniaturised a lot and it will continue to until it meets your hairline and there's little to nothing left on top. If you do nothing to prevent this then, in my opinion, this is inevitable and it will probably happen within the next decade or so, at which point you're going to be left with huge patches of hair missing. Not a good look.

Do some more research into fin, (really take the time to read into actual peer reviewed studies that have taken place over the span of 5, even 10 years with thousands of participants overall involved and their resulting safety profiles etc - don't just listen to completely anonymous anecdotal claims on forums on the internet) ultimately this is your own choice to use it or not, but I know you know your hair loss isn't going to magically stop in it's current state unfortunately. You need to think ahead my friend.

Your loss right now is heading towards a Norwood 5, it may progress further.
You`re only 31. Already you`ve lost a lot of hair in 10 years.

You will need a HT doctor to analyze your donor for miniaturisation and state how many grafts they believe you have available.
If you only have 3000 that can be safely harvested and they proceed to take 4000, then they have overharvested.
They may remove too many from a certain area which will result in overharvesting.

In my own experience, Finasteride has been my saviour. My crown was similar to yours. It was expanding and creeping towards my hairline. Finasteride stopped this from progressing and helped miniaturised hairs grow back strong and healthy.

How many grafts will they place in each cm2? 40 - 50 is average. This will determine how dense your hair will look.

I think one doctor should be fine.

Having an in house consultation first will help you understand what can be achieved if you`re a suitable candidate if you have realistic expectations, and have any questions you have answered directly.

What do you think about Dr. Spammer?

He has mixed reviews and results. Who will do the surgery? Will it be the doctor or a tech? Who is the tech? What experience do they have? Can you view specific results and confirmed reviews from said tech?

I think you should take more time researching. Have some consultations to compare. Ask the doctors about having a HT without any preventative measures in place.

If this is true, why are you considering them?

I im considering them because a lot of guys with afro curly hair have got great results from them and many of them showing the results on youtube. What better way is it to document the results?
I know there can big consequences on choosing the wrong doctor/clinic and that’s the reason i have not done HT. But sometimes is it not worth taking the plunge and still knowing the consequences? Like what is the worst case scenario after HT? Over harvesting donor, scaring and having bald areas and not having more grafts left for another HT. Then i would just shave it all and have SMP to cover scars. Well this already where im at today if i don’t do a HT (shaving or having SMP.

I have read about HT a lot but you have already helped given me very good advices. I have sent my pictures to some clinics and they say around 3000 grafts as you already gave me an idea on grafts amount. Im not scheduling anything with them as im just researching. Also i will schedule with a specialist to have look on my donor etc. Does the specialist have to be a doctor?
Is there any point on having hair loss prevention regiment like shampoos without fin? I have already ordered minoxidil to prevent more hair loss on crown and maybe thicken the crown. How much do you think minoxidil alone will help on my crown as it is today? One of the reasons why im not on fin is because the doctor i had then adviced me against it and in my country it’s not easy to get fin without the doctor approving it plus reading about the side effects. You really have made think about hair loss prevention. I will start with minoxidil for now.

I was most interested to examine my donor with a video microscope but he just had a quick look and said that my donor was good and that most people have a good donor. This doctor was very knowledgeable and made feel at ease but him not examine my donor seemed weird to me. I also checked reviews written by his previous costumers that he only examined donor by photos. Is it normal for a doctor to not examine the donor besides just having a look?

Plenty can and does go wrong. If you go ahead knowing the consequences then there is a chance you will be back here asking for repair options.

SMP can go wrong. Don`t assume it`s a guaranteed solution if your hair transplant is a disaster. Sometimes you can end up beyond repair. No donor, different types of scarring, cobblestoning, nerve damage etc.

I would expect a doctor to examine the donor and scalp thoroughly during an in house consultation.

I have read this thread twice over. There are clear warning signs that raise red flags, I can not fathom out why you are not taking onboard the advice that is being given.
Your emotions and insecurities about your hair are driving you to a rash decision. Take a breather, forget about hair and concentrate on the positive things in your life.

I really appreciate all the advice here. I do not mean to be stubborn. The reason I have not taken hair transplantation before even though I have been interested for a long time is precisely because I have been skeptical and wanted to investigate more.

I've taken all the advice I'm given here, so do not think I do not. I feel like I have come to a point in life where I either take HT or not. I have been researching clinics for many years so I am not completely new to this. I have not been active on HT forum before, so this is me researching more. I guess im just pouring out my crazy thoughts so you guys can give me some advice and another perpective.

Im really interested in checking my donor with a microscope and to have a double take since i don’t want to even consider HT before that.
The last doctor i was with did not analyze my donor with a microscope and said it was good after just having a look. I told him to use a microscope but he said he can already see that my donor is good. There are no other HT doctors where i live. Should i try asking him again to use microscope? Like would you trust the doctor if he does not use microscope to check donor?

Is it only doctors who can analyze donor area or can a dermatalogist do it?

A red flag is that you believe they over harvest.
A dermatologist can check your hair for miniaturisation. A HT doctor is the better option though.
Who was the doctor? You can private message if you don`t wish to name them here.

Have you considered or used any hair loss prevention regimen?

Finasteride, Minoxidil, RU58841 etc.

If you get a hair transplant without stabalising your loss you may need subsequent surgery or quite possibly run out of donor hair.
