Larger FUE case by adding beard hair - Shapiro Medical Group

FUE has become more prevalent over the past few years. There are many good reasons for this including the increase in the number of high quality results. Although, many clinics claim to do very large €œmega € session FUE cases, at Shapiro Medical we have stayed cautious and conservative with our large FUE cases. As we all know, the €œSAFE € donor zone is a defined area and venturing out of that area will indeed give more grafts, albeit grafts that may not last a lifetime. At our clinic, an average €œlarge € FUE case is 2400-2700 grafts. This type of FUE will also typically take 2 days as we do not like to put additional stress on the patient as well as the grafts by doing it all in one day.

I wanted to briefly present a recent case of ours done at SMG where we increased our overall number of grafts by adding beard as donor hair. Typically beard hair has been used as a donor when all of the scalp donor has been depleted. This has been for good reason as beard hair is typically only one or two haired grafts. Also, the characteristics of beard are not the same as scalp. Finally, although beard grafts grow well when transplanted, they typically have a lower yield (growth rate) than scalp donor.

This patient was a 28 y.o. male with a 5+ year history of hair loss. He has not yet started preventative meds. He was a Norwood 4 with a donor density of 68 FU/cm2. This patient wanted as much covered in one session as possible. With his lower density and the patient choosing FUE over FUT (FUT can often get more grafts on the first pass) we looked for additional areas of donor in this patient. Beard was the likely choice. Risk of scarring or hypopigmentation, although still a risk, is not common in the beard. Also, the beard that was used was under the chin, so it is less visible. Because of the increased number of grafts, the patient came into the office for a total of 3 days. The first day was relatively short as only the incisions were made in the scalp. Day 2 and 3 consisted of FUE extraction of the scalp and beard. All the grafts that were extracted each day were transplanted on the same day. We ended up extracting 2600 from the scalp and 499 from the beard for a total of 3099 grafts.

This is a close up of the beard after extractions. The scalp donor delivered a somewhat average distribution of grafts. The beard gave 400 single hairs and 99 double hair grafts. Thankfully, beard hairs are typically thicker than scalp hair so they do help with density. Also, the beard grafts were only placed in the midscalp and used for added density. Beard should not be used in the hairline or other areas that are readily visible.

how many beard grafts does the average patient have?

Every patient is a little different of course so the numbers of total beard hair can vary. The area of extraction can also vary. Typically we will only take grafts from below the chin. It would be possible to also take grafts a little down the neck and even into the beard on the sides of the face if desired.
For us though and in our experience anywhere from 500 -1000 total grafts just below the beard should be possible over a few sessions. Patients with white or light colored beard hair may lower this number as well. The nice thing about the beard is that theoretically you can take all of the hair down there. It is very unlike the donor in the scalp in that you don't need to have hair there.

Why does the beard area leave less to no scarring than the traditional donor area on the scalp?

That is a great question. I am not sure we know the complete answer to that. At our clinic and in personal communication with other FUE docs we have just seen a lower incidence of it. It still is possible and we have to be cautious.

Just like the scalp, although there is indeed scarring in the areas of FUE, the biggest issue I feel is that there is an absence of hair so there is a gap between other hairs. In the scalp this can be noticeable. Also, because there is no hair there is also a decrease in melanocytes that produce pigment in the skin.

One reason that I can see is that perhaps there is less of a contrast when grafts are taken out of the beard vs the scalp.

An interesting factor is skin damage due to UV radiation, as the scalp is exposed to this it may be one of the reasons some people car more than others.

Skin complexion is a characteristic that is affected by many factors. Various genes, nutrition, and environmental factors can play a role in the color of skin. One of the most notable components of skin that contributes to complexion is a pigment known as melanin. Let's discuss melanin and its functions in humans and other animals.
Production and FunctionMelanin is a pigment that is produced by cells known as melanocytes in the skin of most animals, including humans. This pigment comes in different shades, depending on the genetic makeup of the individual. Melanin comes in two basic forms and can range from yellowish-red to dark brown. Eumelanin is the most common form of melanin and is brownish in color. The other basic form is called pheomelanin, which produces reddish-brown color that is often associated with freckles and red hair. The production of melanin in the individual is determined by several factors.
Genetically speaking, every individual on earth has approximately the same number of melanocytes. The difference, then, in the production of melanin is affected by:
  • Exposure to UV radiation: Melanin is produced as a response to UV radiation in order to prevent damage to the DNA in the integument. Individuals, who are exposed to UV light, such as the sun, will produce more melanin for protection. [/*]Genetic makeup: Different ethnicities and cultures are genetically pre-disposed to producing particular shades and amounts of melanin due to inheritance. This is, essentially, one of the primary indicators used in determining race in the human population. It is important to note that this is, and has historically been, a controversial form of human identification. [/*]Size of melanocytes: Melanocyte size varies in different individuals and may lead to a difference in the amount of melanin produced per cell. [/*]Disease conditions: Several diseases may affect melanin production, including albinism, a genetic inability to produce melanin, and vitiligo, a progressive loss of melanocytes. [/*]
These factors all play a role in the production and expression of melanin in the skin. While some factors are more prominent than others, ultimately, they are all important in the final complexion that is exhibited.
Issues With Expression of MelaninIn humans, melanin production is important for the prevention of skin cancers, such as melanoma. This means that the skin becomes darker in most individuals when exposure to sunlight increases. Additionally, overgrowth of melanocytes can lead to the development of moles in the skin. A mole, or nevus, is usually benign (non-threatening) but may become cancerous with increased sunlight exposure.
