Dr Arshad- Hair Dr 1800 Graft Second Pass Surgery and Slight Lowering of Hairline

Hi guys, so I thought I'd make a thread detailing my hair transplant journey so far with the Hair Dr.

I had my first transplant there back in July last year, which consisted of 2000 grafts to the front. As shown below in the pics it didn't exactly go to plan and the density wasn't great (on the left side in particular). After chatting through with the Dr and Sommiya (practice manager) they agreed to redo the areas where growth was looking sparse, and at the same time I paid for an extra 1000 grafts to slightly bring my hairline down. Was originally quoted about 300 grafts to redo the area, however on the day the Dr agreed that more was needed so I think just over 800 grafts were used. They haven't sent over the pics directly post op yet, but I will upload them next week once I have them.

The Dr wasn't entirely sure why the grafts had not taken as well as expected, although possible reasons were the punch size used at the time, which was I believe mainly 0.6mm and probably led to some trouble cleanly extracting anything more than 2 hair grafts. With this op he used a 0.9- 1mm punch.

My donor area wasn't the best (it has strengthened a bit on finasteride), so this probably didn't help things. Dr Arshad was more selective with the hairs that were used from the donor area this time around.

I'm not really going to detail the actual day of the surgery as most of you know the drill already, everyone at the clinic was lovely and the procedure went very smoothly. I've never had a problem getting a question answered either.

Current Meds: Finasteride for the last 15 months, Nizoral shampoo once a week, minoxidil 1x per day- I alternate between the liquid and the foam (will try the tablet version soon after a bit more research), saw palmetto and biotin. No regrowth but the crown hasn't changed in the last year so this seems to be stabilising things.

I was aware when going back that a few patients have had similar complications, although I did think that this was part of the game and that some would always need a second pass to get the results they wanted (Mike's FUE story is a good example of one who went from pretty good to amazing from the 1st to the 2nd procedure). I will say that from spending a lot of time on forums whilst i've been off work this last 2 weeks, it is a bit worrying to see the full volume of patients that are in the same position, but hopefully given that he probably sees over 400 patients a year, these cases are still in the minority.

Anyway I'll keep putting out monthly updates and hopefully will achieve a good result! Does anyone have any tips for helping achieve maximum growth from the grafts?

Hi mate. I got mine done at the same time last year - in a similar boat with regards to growth. It’s good the clinic have done a 2nd procedure. Really hoping this time you get the result you want. Do you have any pictures from your first procedure, pre and post op? Be good to see where your journey started to compare. All the best.

1st Pic is about 2 days after, then two weeks, 10 months uncovered and in bad light, and the last one is how it would look like styled over. The growth was enough so if I had that hairstyle you couldn't tell that I was balding at all, however if it's windy or styled upwards then it becomes obvious very quickly.

Anyway yeah hopefully this time i'll get where I want to be, my hair is quite fine so I'm not the best candiate in the world for a transplant ,the main aim is just want to be able to rock slightly different hairstlyes and not worry about the temples.

Do you have a thread for your journey or any pics? And what's the plan for you moving forward?

That is an awful lot. Stay on the hair loss treatments obviously. You said your donor is not the best. Do you know what you have left, if your hair loss continues. There is a difference having a second op to deal with increased hair loss etc. Not to recover. As said, I really hope you get the best growth this time, look forward to seeing your hair grow.

Thanks mate, i'll let you know in a year! Yes different punch, and he avoided some of the finer hairs this time around and went for the best quality donor bits- so hopefully enough to get to the finish line (for now lol)!

Well they classified my donor area as good for this procedure, and it has certainly thickened after taking fin over the last 15 months. I have also been rubbing minoxidil into the donor area as well just for the hell of it ( i had read a few positive and anecdotal recommendations towards doing this to strengthen patchy donor areas!)
Not sure how many grafts I have left but if this one fails it's most probably game over so I didn't really assess it. Probably 3500 from the back and maybe 500 beard grafts( if I could actually decide to part with them that is!).

I agree it is a huge amount for the area, and yes it does leave far less room for the future, but I know i wouldn't be happy with my hair without a half decent hairline so didn't really have an option.(y):)

Maybe a clinic will chip in how good this is. Something that comes to mind is you will need to continue using because the treatment only works when used. So to keep your donor good and the hair transplanted from the donor good they need meds. I have tried MSM and biotin and find helpful. All the best, good growth

I agree with you. You were not really given any option. For something frankly was not of your doing. So, I do wish you all the best. Using treatment on the donor. Maybe a clinic will chip in how good this is. Something that comes to mind is you will need to continue using because the treatment only works when used. So to keep your donor good and the hair transplanted from the donor good they need meds. I have tried MSM and biotin and find helpful. All the best, good growth
Thanks mate I appreciate it. I take biotin but not MSM, will order some as it can't hurt to be in the routine. Yeah i'll be on the meds for as long as I want any hair, that's for sure! I'll keep posting my progress as I go along, either to show off the results or showcase as a lesson on why it's so essential to find the right surgeon!

Yeah you can see you’ve not achieved optimal results. There is some growth there so I don’t think it’s anything to do with your biology. Your hair is quite light in colour too, I’m not sure if this makes it harder to see the hairs on a picture and there’s actually more growth than what can be seen?

Here’s a link to my thread mate. At my 7 month review, myself and the clinic thought we wouldn’t achieve a good result at all. There was very little growth at that point. I started on topical minoxidil, biotin and saw palmetto and around month 10 Id say I had a lot of growth. Month 12 we decided to begin PRP and oral minoxidil, which I think has contributed to more growth since month 12. My temples are still thin as well as an area on top so we’re going for a top up soon. You can see hairs in those areas, just the density isn’t there.

I’m looking forward to seeing your progress mate.

No worries mate! No I wish, but all the hairs on the photo are the amount you can see in real life.

I've just read through you're thread, your hair actually looks really good! I can see why you want to slightly increase the density in certain areas (particularly the left side), but i would say it's more a case of things didn't succeed as much as you'd hoped, rather than the transplant being a failure. Even with your hair styled up, you don't look like you're balding- just looks like the areas thinner which a lot of guys naturally have anyway.
Are you going to go back to the hair dr for your top-up?

Hello mate, I’m about almost 11 month in and had decent grown on one side but not the other so may have to follow your second. Did they charge you as your hair didn’t take the first time? Cheers

Thanks for sharing @Kelvino1987 - I think it is important that stories like these are on forum, because not everyone gets a "perfect" or satisfactory result and it is important to consider that when opting for surgery. All the best with the results for your second surgery. I am looking forward to following this thread and really hope you are happy with the outcome this time.

Do you have any update pictures mate? This is me at pre op and 10 months from the same clinic. I had 3000 grafts in total.

Hello mate. I’m doin the same as the left side of mine hasn’t come through as much as the rest. Think I’ll be also adding some to the crown and top of my head
