Advice on potentail hair transplant

Hello,

I am a new member of the forum. Thanks you very much for all of you, I have now I think a best understanding on Ht and the evolution of Alopecia with the help of the comments I saw here.

I am 45 years old and I take minox since 20. I think I am Norwood 4 and I am thinking about Ht. I think I drop my hair slowly, but I saw somewhere that the more agressive part is at 50-60 …

I would like to get a mature hairline as I have never had a perfect hairline.

I saw a surgeon that suggest me 1000-1200 grafts for golfs and a little topee hair but I'm afraid that's will not be enought. So I would like to have your advice... Whet do you think about it ?

Hi @arthurSam, at your age and hair loss pattern you are a good hair transplant candidate. Generally speaking, aggressive hair loss is normally at an earlier age. That said, you could obviously continue to thin. Albeit slowly and without any great change in your hair growth pattern. As a result, keep up with the minox, especially if considering a hair transplant.

A slightly conservative approach would be ideal for you. The idea is to frame your face better and remove the deep recession. Without turning the clock back to when you were twenty. But you don´t want to go too high otherwise it still highlights the recession rather than framing your face properly. The numbers are a conclusion of the area to cover and your hair characteristics. Having to go back into your existing hair to create a seamless blend. You have medium-coarse hair and a lower contrast between your hair and skin complexion. As a result, probably around 1500 or slightly more would give you a pleasing design and go back into your existing hair. Hope that helps, best wishes with your research.

Thanks you very much for your answer. Some people said that I must use 2000 graft... so between 1500-2000 seems a good choice. I will see if the surgeon I saw think it is possible.

It really depends on the ethos of the doctor. The idea is to place enough hair density to create a natural-looking result that blends with your existing hair. This is partly achieved by blocking the scalp from being seen through your hair. It´s possible to use 2000 plus in the same area that say 1500 or so would be sufficient. Assuming it all grew it would obviously be thicker but whether it is necessary is debatable. Ideally, you want to create a natural result with minimal change to the donor. Ensuring if your hair loss does progress you can maintain the placed hair density from the initial transplant and treat the new area of hair loss.

The clinic doe not want to use more than 1200 they said that is for protecting from shock loss... I am a bit confused because I saw other surgeon who proposed me 2000-2500 but for a bigger area with a front line lower and less mature ... But I don't want to change radically so I would prefer the mature one. But they want only do the big one...

Hi @arthurSam, shock loss is unpredictable but should not be common. For you, this is a normal hairline rebuild, unless there are issues not stated. There would be no indication why shock loss would be a concern. Assuming all surgical care is taken. In your case, unsure why increasing the numbers would increase the risk of shock loss though. As the increase in graft numbers would be minimal and along the hairline rather than within your existing hair. That said, 2-2500 are large unnecessary numbers for what you are looking to sensibly achieve.

Some suggest using minoxidil before, then stopping for your procedure and starting again after. To help reduce the risk of shock loss occurring. As you already use it, this should be considered a benefit. Although you should stop prior and start again after your procedure. But your Dr will give you specifics.

Maybe ask the Dr to confirm the hairline design. If you are happy with that and the clinic is also happy a natural-looking fullness can be achieved. Then all is well. But, best not to compromise. While it is possible to add more hair later to a hairline. It is not ideal. Often the entire hairline needs some work. Albeit minimal. Requires a lot of grafts that would not have been required if designed that way initially. Best wishes

Which Dr did you see, is that the white line there? It looks ok but pretty conservative.

Somewhere around 1500-2000 grafts would do good depending how aggressive you want to go.

I would not lower the hairline at all, just fill in the receded part and preferably see a Dr who can dense pack it with 50+ grafts per cm2 so you just need one surgery and not two.

I think the hairline which is red is proposed by someone who does not know what they are doing. I would be very cautious to doing the temples, they are very difficult to do and I think you dont need it.

If you are in the UK you should have a consultation with Dr Edward Ball, Dr Mark Tam and Dr Raghu Reddy and see which idea you find most appealing for your situation between these 3 consultations. You can just send an email to all and they should answer you in a week or so.

If you feel like travelling there is lots of clinics out there in Europe who do good work. Its best to have lots of consultations.

Hello Arthur. Welcome to the forum. I note you are using topical minoxidil since the age of 20. I take it your hair loss has slowly progressed since you began your treatment. May I ask, why you don’t take Finasteride? Also, do you have any pictures showing your hairline all those years ago?

At 45, I think a conservative age appropriate would look good on you. The drawing that Louii provided above is what I would suggest. Maybe a little less acute going towards the temple corners. I`d also say 1500 grafts to a possible 2000 depending on how much miniaturised hair you have in the forelock and transitioned zones. Have a look at this patient who has a similar hair loss pattern to you and had 1500 grafts. Hair transplant

Did the doctors you consulted with check your donor and advise on its condition and number of grafts available?
Did they both say FUE?

Last doctor I saw said that I get a good donor zone and if I take 2500 grafts now it will be OK for later if needed... But he said that I got a pattern and normally I will not go bald in vertex.

Next week I'll see another clinic which make tests and I will ask for miniaturisation. For the moment none had say anything about miniaturisation, and I never asked because I dont know what it is... For what I understand now, it can predict if vertex will go bald some years later ?

All doctors proposed FUE for me.

Next week I'll see another clinic wich make tests and I will ask for miniaturisation. For the moment none had say anything about miniaturisation, and I never asked because I dont know what it is... For what I understand now, it can predict if vertex will be galbed some year latter ?

All doctors proposed FUE for me.

Be cautious who you choose to go forward with, there is 95% con artists out there and 5% is actually doing great work. Take your time and do research and understand all the parts what is important with a hair transplant.

Miniaturisation is when hair follicles begin producing weak shrinking strands of hair that usually leads to hair loss. The hair shaft will be thinner and shorter in growth.
Predicting you won’t go bald in the crown when you have lost quite a bit of the fontal hair must be a psychic power they possess. Which clinic said this?

I see. Well it is pretty hard to predict and you’re at a higher risk because of your hair loss you’ve suffered already.
