Hi All,

Our patient was a Norwood 6, average groupings with fine hair; his goal was to achieve total coverage if possible. With his age, a low hairline was not important so it was designed in accordance with his hair loss pattern and his facial bone structure to give a natural framing of his face.

Two procedures were performed over two consecutive years to achieve his result, the first concentrating on his hairline, frontal third and midsection with light coverage in the crown and the second to reinforce the density in the midsection and the crown.

The first procedure was 4442 grafts and the second 2863 FU ´s.

Total graft count being:

1 Hair 2 Hair 3 Hair 4 Hair

1298 4086 1366 555

Total 15,788 hairs and an average of 2.16 hairs per follicular unit, slightly under average.

Guessing from the new goatee, this has really improved his self image. So natural - I think that's the most important thing.

Thanks to all for their comments, as can be expected our patient is very pleased with the transformation and overall the naturalness of his result.

Hi bullit,

Yes, a great deal of our work are larger sessions, 4000 plus, we feel, when possible, and obviously with planning it ´s the way to achieve the maximum result, for a simple reason the donor is virgin at the time so will always be in the optimum condition to harvest grafts, thereafter future sessions will be smaller, so donor management is vital whether a lower Norwood or high like our patient here to ensure the goals can be met.

This patient has exactly the same type of hair as me which makes it very interesting for me.

Hattingen: Do you know approximately how many grafts were placed from...

1) The hairline to the back of his ears

2) The back of his ears to the very bottom of the crown

I know you can't give me an exact number and I know that we all have differences when it comes to hair groupings etc., but it would be interesting to know if "section nr 2" needed quite many more grafts since the crown tend to need many grafts. It would also be very interesting for me to know how many grafts "section nr 1" needed because this is the section that - for me - is the most important since it frames the face. His result looks dense although he has fine hair.

Anyway, great result as always, but am I mistaking by saying he is more like a NW5?

Hi Iniesta,

It ´s hard to be too accurate I am afraid, but as a guide; the frontal third will normally take around 3000 grafts but due to the conservative hair line design we were able to restore the area with less than that and maintain a good density, this allowed for greater coverage and maintain density as we worked back. The first procedure 4442 being able to give a light cover over the crown and then the second procedure reinforcing the mid section and then adding greater density to the crown; if we dropped the hair line lower and wanted to achieve full coverage the graft numbers would have easily had to increase by 1000 grafts, partly due to the hair line and also the crown needing more.

His sides are still relatively high and this also helped with the distribution of grafts although his crown had dipped quite low, hence we categories him as a NW6, the picture below we would say the blue line would represent a typical NW5 whereas you can see in reality his crown was much lower. This was all taken into consideration when discussing the redistribution design and plan with our patient.
