Hi, i'm 40 and i am norwood 5. I want to have an HT and i am wondering if i can restore my problem with one session. Is it possible?

if you have enough donor you could get a mega session which will give you thin coverage over the whole top of your head, you got any pictures to share.

Personally i would opt for a few smaller sessions and reevaluate after each one....you may find that you would prefer to have more density in specific areas like the hairline and if you use all your donor in one session getting thin coverage all over then future tweaks may not be possible unless you incoperate body hair.
The planning is crucial if your a norwood 5.

In answer to your question though BM is correct it is possible if you have sufficient donor to get a mega session.

I agree with bullitnut. That was the approach suggested by the doctors in my case, even though I insisted that I wanted to get it over with my second HT. Eventually I believe that this approach was correct. Be aware that it is likely that in such NWs you might need hairs from your body and / or beard. In my case I had used some and really helped me a lot.

To a large extent it is going to depend on your hair and skin characteristics, technique used how large a session can safely be carried out and the coverage that can be achieved, with obviously a natural, if not full looking density from hair line to crown.

Your expectations are very important to be realistic, for example a high NW is probably not going to be able to have a low flat hair line and still have total coverage. The higher the NW planning is obviously easier as the hair loss pattern has progressed and easily seen.

Your donor at present is virgin, never been touched surgically, in this state you have the ability to harvest the most hair in one procedure, any procedures after that the donor area is no longer virgin so less hair. So, in cases of a high NW when maximum coverage is desired it makes sense (safely) to go for the maximum number in the first procedure. Hair density, laxity may only allow say 4000, although with good characteristics this number could be much higher, 6000 say. Then knowing what can be achieved in the first procedure you can plan hair line placement in relation to coverage. Possibly opt for a higher density in the frontal and mid section, leaving the crown, or gradually dropping the density in the mid section to give a coverage over the crown, giving the option later to increase crown density if you wish, but at the same time if you are happy with total coverage and less crown density you have a result in one pass. Always assuming you can reach the numbers required in the first procedure to give you those options.

Yes all Norwood 6 patients will require more than one procedure unless their expectations are modest. That said a mature hairline open temples which would be age appropriate and a light coverage on the crown is really what you have to accept.

The Norwood 6's who can get more than this have all 3 of the following, very good density into the donor, excellent hair characteristics and excellent laxity (strip only).

In your situation your donor is the key as to whether you should go ahead or stick with what you have a continue to shave as I feel from the photo that it suits you and you have a decent head shape to do it.

I would personally need to see some clear photos of the donor ideally with a little more length before advising further as to what you should do.

Ideally it is best to treat an area in one pass, for example the hair line there should be no need to go back to make it look natural, and in larger cases, the frontal third also. They should be given priority in respect of density, then going further back through the mid-section and crown it maybe necessary to increase density if the first operation could not deliver enough graft numbers.

Increasing density should be relative to the donor supply, patient goals and what is achievable, but what should take priority is in a single procedure the person is left with a natural result from hair line back, spreading hair evenly at a modest density throughout does not create a natural result for almost any HT candidate.

Dear Siolas,
While it is true that there are various approaches to a case like yours, its important too for you to make your own decision. Let me explain.

It is possible to
1. Do a hybrid procedure (strip + FUSE/fue from head and beard) to maximise number of grafts
2. Less is more approach using FUSE/fue alone from scalp, beard and body
3. Go for multiple sessions using strip first and FUSE/fue later

Examples
11,000+ grafts in one visit

Conservative but dense hairline approach

Body hair grafts alone - 8000+ for a NW6

Youthful hairline based approach (strip for a NW 6)

Less is more approach for a NW 7

There are as many solutions as people. See a lot of results and decide for yourself.

If you have queries, please do not hesitate to email at [email protected] or post them here.
Regards,
Dr. A

Thank you Mr garageland, Flanders35, hattingen, Dr Arvind for your response. I understand now that I will have to restore a single area first €¦ probably my a zone €¦ and in a second session to continue with the b zone.. etc..
FUE is most probably my selection as I shave my head now..i will probably shave my head in the future..don €™t wanna risk that.
I will do my research, see about prices etc. and let you know. I really appreciate your help.
Thank you again.

Remember Fue can leave scarring too if you want to shave down in the future.

Dear Siolas,

If your are looking for maximum density, its better to restore your a & b zone in a session now and have a second session for your crown (after a year).

This a safer approach for your donor area and for a natural result.

For more information don't hesitate to contact dhiglobal.com.

Hi Siolas,

As BM said regarding shaving down, FUE scars also and no one can guarantee that it will not be seen or look man-made due to the pattern of the extraction.

Also you need to think about the cosmetic aspect, a hair transplant works greatly on the principle of creating the illusion of density. This is achieved partly by the density placed and largely by the length of hair worn, donor and recipient.

The shorter the hair the less illusion of density because each hair needs to lay over the next to block the reflection of light off the scalp. Also the orientation of the hair can alter if you wish to keep a very short hair style, this can be "designed" when the slits are made to lay more acute but even so a very short hair cut still will leave the result looking a little thin; hence you rarely see very short hair cuts regardless of the technique used.

Dear Siolas,

2.500 grafts will be enough for a natural result. About short hair, the wounds €™ healing is rapid, about 3-4 days, and do not leave any visible scars or marks. (punches of 0.7mm and 0.5mm in diameter.) Their use is depending on the diameter of the hair follicle, the shape and the direction of the follicles inside the dermis which is according to the kind of hair (curly or straight).

I aggre with you about the illusion part.

Thank you for your help. As i travel between London-Athens very often, i had the chance to visit the DHI Clinic. I saw a guy that had an HT 9 months ago and watched and 2 sec surgery and ..they were very good. The bad news is that the results I saw was what we call "easy cases". Both had small problem. Dr Papanikolaou recommended 2500 grafts for my a and b zone and the rest of the head after 9 months. I will get a 2 day session. I will keep you posted. Peggy I want to thank you more for answering all of my queistions through email.
