Pictures showing FUE extraction site healing

We took some interesting pictures, using our electronic microscope.
Please note that the pictures below were made with about 60* magnification.
Extraction was done with a 0.80 mm special FUE punch, titanium coated.

Thanks for posting this, the punch hole is healing extremely quick.

Bverotti, would you say the quick healing is due to punch size.

In My opinion:
Yes, punch size does matter.
But for hair restoration it is not the smaller the better.
Too large and you do needles tissue damage. Too small and you can't get the entire grafts out.
So for caucasian men we believe the ideal size is 0.7-0.8 mm.

I also believe manual extractions are the way to go. Some devices rotate at such high speed that due to friction you will create heat into the surrounding tissue. This could cause tissue and follicle damage.

Thanks for the photos.

We would be of the same persuasion, I know clinics all have different protocols for tools, size and type of punch, how many grafts a day to do etc but with regards to punch size and type we would agree with you.

Too small and there is a danger of not having enough protective tissue on the graft or as you say will not be able to extract intact the whole follicular unit, more so for the larger groupings in the occipital area. Too large then arguably more damage and possible damage to surrounding follicular units also, more visible scarring etc.

Manual is not only about less heat and trauma generated etc but also for the doctor to feel more what he is doing, more direct connection with the patient, more feel for what is happening under the skin as he punches.

I've only had a small manual punch used on me and after many surgeries and 7,000 scalp FUE and 1,400 beard I have a good donor and wear my hair at 2.5mm with no surgery signs, this is attested to by many who have met and examined me in the flesh. I do not personally think I would have the same donor if I had been operated on with larger motorised punches. Personal opinion only of course but after 7 surgeries an opinion that I am entitled to.

I think it all comes down to something we can all relate to regardless of our occupation. We can concentrate on how it can be done better even if that means slower and with higher difficulty or we can choose to do it faster and maybe just because it €™s easier.

Faster and/or easier is okay with some types of work I guess but not with cosmetic surgery in my opinion.

Not to take the thread off topic but is that some special type of camera lens?

...in this particular instance... there is NO electronic microscope here... This series of pictures was performed by a simple "light" microscope... and is definitely not an "an electron microscope...

So in my opinion... c;;9

Next time I will try to use an electron microscope and show the healing process at atom level :)

Seriously, it is just a USB camera with high magnification, anyone can buy them online.

Never mind, you get the idea haha,

Have you seen any photos or can provide any photos you took of the damaged tissue and follicles caused by the friction of rotating motor punches?

Here is a $450,000 USD electron microscope... lol I was busting your chops Bverotti. It would be awesome if someone could use this type of technology to take pictures of FUE holes.

Hi Jana,
A couple of years ago we where a test bed for some electric punch manufacturers.
They send us their equipment for testing.

We noticed that:
The extraction feeling is lost due to the weight and vibration of the device. The feelings which is developed is key to good extraction.
The rednes surrounding the extractions where red for a longer period of time as opposed to the manual extractions.

So clearly there was more skin dammage and also the hair per graft count was much less.
We did not take pictures, nor did we do extensive trial. It was clear to us very quickly that manual extractions are key, and there is nothing that can replace the human finger tip feeling, not even close.

In my mind I would imagine it is almost like comparing a player piano with a great pianist. It kind of sounds the same but it €™s not. When the instrument is played by hand the dynamic force required which constantly varies, controls the dynamics of the performance.

Anyone can buy a player piano but can anyone be a Mozart or Beethoven?

Some electronic punch manuacturer? I'm inclined to respect the instrumentations devised by well known and respected physicans than some manufacturer. There are countless variations of motorized punches out there and the speeds of the punch are controlled with most of them. Dr. Cole currently has a motorized system that's very delicate, sharp punch that goes through the skin like butter. We've seen donor and healing from many sharp motor and non motorized punches. Have to say I've never noticed any more bleeding or that they took longer to heal. When I visited number of clinics few years ago who all used sharp punches, I was most impressed with the quality of grafts from their motorized punches.

I'd have to disagree with you that you cannot replace the human finger tip feeling...there can also be more unintended movements and fatigue error in human hands. There are benefits you can attain with motorized punches that you cannot get with human finger tips as well. Consistency and speed being the two main benefits.

We don't use sharp punches - motor or non motorized. So that's a different debate altogether - sharp vs blunt.

In my opinion... In a well skilled hand a motorized punch can become a huge advantage over strictly using manual. However at the same time not all motorized FUE machines are good in all hands.

We tested several motorized FUE machines. Several manufactures and physicians sent us machines to test. We settled on a simple rotary device using a blunt punch at 0.8mm to 1.0mm constructed by Dr. Harris and called the SAFE SYSTEM. For the past three years our efficiency increased and the transection rates went below our already low transection rates.

In my opinion while there is a need know how to do manual extractions... there is also a need to become more advanced as you become experienced.
