This case involves a Norwood Class V–VI patient with a good-quality donor area.
Due to the extent of hair loss, we planned two FUE sessions to ensure optimal coverage and long-term donor management.
In the first session, 5,000 grafts were transplanted to restore the front, top, and mid-scalp areas. The crown was intentionally left for a second planned FUE session, estimated at approximately 3,500 grafts.
Hairline design is a key factor in achieving a natural and sustainable result. In this case, the hairline was designed slightly above the patient’s remaining native frontal hair, allowing for a balanced, appropriate appearance while preserving grafts for future needs.
For graft extraction, we distributed the punching evenly across the entire donor area, using small punches, shallow scoring, and two-forceps extraction. This technique minimizes trauma and scarring while preserving donor density. The procedure was spread over two days, ensuring optimal donor recovery and maintaining the ability to return safely for additional graft harvesting in the future.
In the recipient area, single‑hair grafts were used in the frontal zone, and sapphire blades were employed to create precise incisions with minimal trauma. Careful attention was given to direction, angle, and implantation pattern, allowing the transplanted hair to closely mimic natural growth and appearance.
